Minister application:     What about Ordination?

*  All questions must be answered and sent along with the appropriate fee and recent photo of yourself sent to the Secretary of the FCMI.

*  Complete "Application for Ministerial Affiliation."  Application-Individual.pdf 

*  Complete "Application for Ordination Request." Ordination Application.pdf

Church application:

Group A:  (Churches without a Constitution)

*  Complete "Application for Church Affiliation."  Church-Application.txt  or  Application for FCMI Church Affiliation.pdf    [Get Adobe Reader]

*  Send two copies of Minimum Church Constitution.  Minimum-Church-Constitution.txt

*  Application fee (See top of "Application for Church Affiliation."

Group B:  (Churches with a Constitution)

*  Complete "Application for Church Affiliation."   Church-Application.txt  or  Application for FCMI Church Affiliation.pdf     [Get Adobe Reader]

*  Send two copies of your church constitution.
